Add Activity detail section in New Assistance page

Add Activity details section in New assistance if Activity is applied.

There will be 3 fields:

Description (text field like notes in BNF detail) (for all targets) Households Targeted - number (for Institutions and Communities only) Individuals targeted - number (for Institutions and Communities only) Send these new data in Save button in activity parameter
